Form privacy information

Information you send

The service form requests your name, phone number, email, service address, system type, and description of the issue. The Repair Reward form also requests your approximate repair date, repair details, and a paid invoice.

How requests are handled

These forms use FormSubmit, a third-party form-processing service, to route your request to info@forestprohvac.ca. Forest Pro uses the information to contact you about your service or eligibility request. Submitting the form does not subscribe you to marketing messages.

Invoice uploads

Only send the information needed to review your HVAC repair. Remove payment-card numbers, bank details, and unrelated personal information before uploading. Accepted formats are PDF, JPG, and PNG, with a 5 MB limit. Uploaded files are sent through FormSubmit; this website does not provide a customer document-storage account.
